Catholic Charities of Fairfield County is seeking a compassionate and motivated Housing Case Manager to join our Connecticut Collaborative Reentry (CCR) housing program located in Stratford, CT. This full-time (35 hrs./week), benefits-eligible position provides an opportunity to support individuals by helping them achieve housing stability, employment, and long-term success.
Key Responsibilities
•
Provide strengths-based case management services.
•
Complete client assessments and develop individualized service plans.
•
Connect clients with community resources and referrals.
•
Assist with job readiness and employment support.
•
Advocate for client needs and coordinate services.
•
Maintain accurate documentation, data entry, and required reports.
•
Ensure compliance with supportive housing policies and federal regulations.
Qualifications
•
Bachelor’s degree in human services, Social Work, Criminal Justice, or a related field or High School Diploma/GED with relevant experience.
•
Experience working with formerly incarcerated individuals is preferred.
•
Knowledge of supportive housing practices and applicable federal regulations.
•
Strong communication, organization, and problem-solving skills.
•
Experience with data management and reporting.
•
Bi-lingual preferred.
•
Valid driver’s license, reliable vehicle, and current registration and insurance required.
